About The Position

This is a per-diem position for Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) within an Adult Day Program. The role involves providing direct medical care, supervising other nursing staff, developing and implementing care plans, managing medications, and ensuring compliance with regulations. The position requires a Massachusetts license, experience with elders and adults with physical or cognitive impairments, and the ability to work cooperatively within a multidisciplinary team. Some supervisory experience is preferred.

Requirements

  • Registered Nurse, or LPN licensed in Massachusetts.
  • Minimum of two years’ experience working with elders and adults with physical or cognitive impairments.
  • Sensitive to the needs of elders of all socioeconomic backgrounds.
  • Ability to work cooperatively as a multidisciplinary team leader.
  • Knowledge of good body mechanics is required.

Nice To Haves

  • Some supervisory experience with other nurses and direct care staff preferred.

Responsibilities

  • Evaluate prospective participants for admission, assist in obtaining and reviewing medical information, and complete necessary documentation for Mass Health Authorization.
  • Supervise the admission of new participants, including physical assessments, electronic and paper documentation, and initiating plans for medication administration and care.
  • Develop, implement, coordinate, and evaluate the nursing components of multi-disciplinary care plans, including yearly assessments and renewals.
  • Generate and supervise the generation of comprehensive MD orders for participants and ensure they are signed and returned.
  • Maintain all medications, med documentation, administration, and glucose monitoring in compliance with DPH and Mass Health regulations.
  • Supervise the writing of monthly nursing notes, emergent/ongoing issue notes, and incident reports.
  • Provide direct medical care and supervise direct care by other nurses, including assessment of emergent issues, individual counseling, medication administration, vital sign monitoring, and coordination of care with various providers.
  • Supervise Personal Assistants (PAs) and assist participants with Activities of Daily Living (ADLs), including toileting, incontinence care, ambulation, transfer, and eating, requiring knowledge of good body mechanics and occasional lifting.
  • Provide or supervise health groups for participants.
  • Work with the Kitchen Coordinator to ensure meals meet dietary requirements and review daily menus.
  • Participate in serving lunch daily.
  • Oversee all nursing documentation and requirements for Mass Health and SCO authorizations.
  • Work with Compliance to train and supervise on OSHA regulations and other health and safety standards.
  • Coordinate with other disciplines to provide training and orientation for staff on various health and safety topics.
  • Maintain a program of Nursing Competencies and the Nursing portion of PA Competencies, including initial training and yearly reviews.
  • Supervise nursing and medical students.
  • Review nursing time sheets and submit them for approval.
  • Attend relevant meetings, including clinical, Operations, Compliance, Regulations, and MADSA.
  • Monitor medical supplies and order as needed.
  • Collaborate with clinical team, Program Director, Admissions Coordinator, CEO, CAO, and Funding and Communications personnel for client care or operational issues.
  • Agree to work as assigned, potentially outside of regularly scheduled hours/days with advance notice when possible.
