About The Position

This per-diem position in an Adult Day Program involves providing direct medical care, supervision, and coordination of nursing services for elders and adults with physical or cognitive impairments. The role requires evaluating prospective participants, developing and implementing care plans, managing medications, and ensuring compliance with regulations. The position also involves supervising other nursing staff, assisting participants with daily living activities, and collaborating with a multidisciplinary team and external providers.

Requirements

  • Registered Nurse, or LPN licensed in Massachusetts.
  • Minimum of two years’ experience working with elders and adults with physical or cognitive impairments.
  • Sensitive to the needs of elders of all socioeconomic backgrounds.
  • Ability to work cooperatively as a multidisciplinary team leader.
  • Knowledge of good body mechanics is required.

  • Some supervisory experience with other nurses and direct care staff preferred.

Responsibilities

  • Evaluate prospective participants for admission to the Program and assist in obtaining and reviewing their medical information.
  • Fill out all necessary documentation requiring RN license for Initial Mass Health Authorization process.
  • Assume responsibility for (and supervise) the admission of all new participants to the program, including physical assessments, implementation of all necessary documentation (both electronic and paper), initiating a plan for med administration if needed and all other elements of a plan of care during their days of attendance.
  • Develop, implement, coordinate and evaluate the nursing components of a multi-disciplinary care plan for each participant.
  • Review as required in collaboration with the clinical team, complete yearly assessments and renew care plans yearly.
  • Generate (and supervise other nurses in doing so) comprehensive MD orders for each participant upon admission and every 6 months for PCP’s to sign and return so that nursing continues to operate under current MD orders as per regulations.
  • Maintain all medications, med documentation and administration and glucose monitoring in compliance with DPH and Mass Health regulations.
  • Assume responsibility and/or supervise the writing of monthly nursing notes on all program participants, any nursing notes needed for any emergent or ongoing issues and any incident reports.
  • Provide direct medical care (and supervise direct care by other nurses) for all participants including assessment of emergent issues with any follow through needed, individual counseling on nutrition and health issues, administration of medication, taking of vital signs with frequency as ordered by PCP, coordination of care with all other care providers such as family, community agencies, case managers, HHA’s and VNA at home, PT’s (in program and at home), PCP’s and MD specialists, and the clinical team in ADHP.
  • Supervise the PA’s and assist the participants with any Activities of Daily Living, including toileting, incontinence care, ambulation, transfer and eating.
  • Provide or supervise health groups for participants as needed.
  • Work directly (and supervise other nurses) with the Kitchen Coordinator in assessing that all meals meet dietary requirements, both in general under regulations and individually for each participant’s medical needs.
  • Review daily menus to provide guidelines for the serving of lunch.
  • Participate in serving lunch daily.
  • Assume responsibility and oversight of all documentation and requirements related to nursing regarding the Mass Health and SCO authorizations for all participants on a monthly basis to ensure they are done comprehensively and in a timely manner as per regulations.
  • Work with Compliance to train and supervise on all issues regarding OSHA regulations and other health and safety standards, and on other issues related to nursing.
  • Coordinate with other disciplines in ADHP to provide training and orientation for staff in areas such as health issues in the elderly and disabled populations, body mechanics in transferring and ambulation, personal care, assistance with ADL’s, therapeutic communication and behavioral issues.
  • Responsible to maintain program of Nursing Competencies as well as the Nursing portion of the PA Competencies, initial training and yearly reviews.
  • Responsible to supervise nursing and medical students.
  • Review nursing time sheets and submit them for approval every 2 weeks.
  • Attendance at all relevant meetings, including clinical, Operations, Compliance, Regulations, MADSA and all others that apply to the Nursing Management Role.
  • Monitor medical supplies and order as needed.
  • Collaborate with clinical team, Program Director, Admissions Coordinator, CEO, CAO, Funding and Communications personnel of Rogerson Communities for any emergent or policy issues that arise regarding client care or ADHP Operations.
  • Agree to work as assigned.

Benefits

  • Compensation: LPNs $32/hr- $37/hour DOE, RNs: $34-$42/hour DOE
