Requirements

  • Registered Nurse with valid certification and experience working persons diagnosed with substance use and or co-occurring (mental health) issues is preferred.
  • Licensed Practical Nurse with valid Illinois license and experienced working persons diagnosed with substance use and or co-occurring (mental health) issues will be considered.
  • Community case management experience is a plus
  • Ability to promote positive behavior and manage resistant behavior.
  • Physical ability to safely and effectively manage aggressive behavior.
  • Assertiveness, confidence and leadership abilities.
  • Effective oral and written communication skills.
  • Effective time management skills.
  • At least twenty-one (21) years of age
  • Must have a valid Driver's License
  • Physical exam that includes TB and drug testing.
  • Must pass both federal and state background checks which screen for crimes including, but not limited to drug related crimes, violent crimes, sexual abuse, domestic violence, child abuse, and repeated misdemeanors.
  • Satisfactory completion of background screening and applicable pre-employment checks, including but not limited to employment and/or personal reference and driving records.
  • Ability to work with computers and the necessary software typically used by the department.

Responsibilities

  • Provide direct service to members. This includes but is not limited to group education and counseling, medical case management and crisis intervention as required. This may be done in the community or on site at a ARCH location. Medical assessment, health education and follow-up with physicians and psychiatrists as needed. Monitor medical needs of ARCH clients.
  • Intervention with collateral sources, agencies, bureaucracies, facilities, and families on the member’s behalf.
  • Nurse will be responsible for the medical caseload of all (currently 49) members as assigned, coordination of treatment and treatment planning and the development and implementation of individualized service plans in collaboration with treatment team.
  • Each member requires the development and implementation of a Service Plan which addresses the members’ health needs according to their medical goals. Each goal is to be addressed with objectives and techniques for accomplishing each goal. This may require developing resources either within the organization or within the community as required by your supervisor.
  • Service plans are to be developed in cooperation with the members, their families, collaborating agencies and others as necessary. Service Plan development includes but is not limited to a comprehensive assessment of the members’ function levels and community medical supports and contact with collateral treatment providers.
  • As crisis intervention may extend outside or occur after regularly scheduled program hours, you are expected to provide the required services necessary to manage these incidents. Nurse may also be required to be on call to provide for after-hour coverage.
  • When members are hospitalized, the nurse is to work cooperatively with inpatient hospital staff, keeping logs and other records of all contacts as required by agency practice or supervisor.
  • The ARCH Program is committed to providing a wide variety of opportunities, activities, social events and celebrations for the people it serves. You are expected to participate, attend and provide support and service to members on such occasions as required by the supervisor.
  • The nurse is responsible for finishing all assigned paperwork completely and on time. This may include but is not limited to: reporting data to MIS or Research, reporting services delivered, writing case notes and or reports, member evaluations and other internal and external reports. The evaluation of job performance includes the quality and timeliness of written work.
  • Attendance is required at a variety of meetings as assigned by supervisor. This includes but is not limited to staff meetings and clinical team meetings.
  • Provide accurate and timely information to your team and supervisor on all topics and issues related to the operation of the agency and the well-being of its members.
  • Ability to function in a computer-oriented office, including working knowledge of pertinent software.
