About The Position

The RN Care Manager has 24-hour accountability for the care of all residents assigned to his/her units. The RCM’s name will appear on the residents’ medical record and on the care plan. The RCM collaborates with all members of the healthcare team to develop and maintain the highest quality of nursing care for his/her assigned residents. The RCM works under the direction of the Director of Nursing Services.

Requirements

  • Completion of Accredited Licensed Nursing Program (RN)
  • Minimum of 1 year professional managerial experience in related field
  • Clinical observation and assessment skills
  • Proficient computer skills necessary to complete documentation, review electronic health records and communicate with team members
  • Current RN license
  • Current CPR certification

Responsibilities

  • Adheres to state regulations concerning delivery of resident care and assures that services are provided and documented in the medical record.
  • Assures that resident’s admission status is assessed and documented in the medical record within the first 8 hours of admission.
  • Assures the need for nursing services is assessed to include restorative and documented in the medical record within 14 days of admission.
  • Assures that a plan of care for nursing services is developed and documented within 24 hours of admission and completed within 14 days.
  • Assures that residents’ condition is updated and reviewed when resident’s condition changes and no less often than quarterly.
  • Assists during the annual State Survey process.
  • Evaluates nursing delivery by administering resident care audits and unit rounds on a daily basis.
  • Implements plan of care and assure that goals are measurable, realistic and consider the wishes of the resident and family.
  • Carries out physician orders ensuring they are written according to facility policies and procedures.
  • Directs resident care conferences.
  • Participates in the quality improvement process.
  • Establishes priorities for resident care and delegates as necessary to charge nurse, and CNA.
  • Accurately assesses residents’ physical and psychosocial status.
  • Reports changes in resident condition to appropriate personnel (charge nurse, DNS, MD, RD).
  • Identifies resident illness (via labs and signs/symptoms, etc.) and uses necessary resources for unfamiliar conditions, altering care plan to reflect changes in resident condition.
  • Responsible for the initiation and discontinuation of alert and Medicare charting.
  • Provides resident and family with explanations of conditions as necessary.
  • Implements MD orders timely with understanding of purpose of order as it related to resident’s condition.
  • Coordinates provision of services with other health care providers (MD, PT, SS, Activities, etc.).
  • Keeps work area neat and coordinate resident care with charge nurse providing instruction and supervision as needed.
  • Establishes priorities for resident care; participates and delegates as necessary.
  • Assists with staffing needs ensuring required staff are on duty and obtains nursing staff coverage in case of illness or absence in collaboration with staffing coordinator.
  • Utilizes problem-solving principles in resident care delivery.
  • Investigates nursing personnel incidents, accidents and errors initiating corrective actions as needed.
  • Supervises charge nurse.
  • Conducts unit meetings as needed.
  • Participates with hiring and orientation of new personnel and counsels as needed.
  • Recognizes legal responsibilities and functions within scope of practice.
  • Demonstrates professional responsibility for attendance record and professional conduct.
  • Provides input to changes processes and supports the DNS and NHA in change process.
  • May provide coverage in DNS absence.
  • Interprets and adheres to established policies and procedures of facility.
  • Accurately assesses and documents care required for the resident.
  • Reports problems, needs and concerns to DNS.
  • Assures that the 24-hour written shift report is completed for respective unit.
  • Interacts with residents, families and all members of the health care team in a professional manner.
  • Participates in infection control procedures.
  • Assures accurate completion of the resident chart along with Medical Records personnel.
  • Acts as a resource person and accepts guidance from available resources.
  • Attends facility and nursing department meetings as scheduled providing recommendations for policy and procedure development and revisions.
  • Delegates pertinent nursing tasks to RN/LPN, CNA’s within their scope of practice.
  • Ensures that orders involving other departments are communicated and implemented per facility policy.
  • Participates in in-service education programs.
  • Attends outside conferences and communications information to team members.
  • Serves on nursing committees as appropriate.
  • Responsible for meeting own continuing education requirements.
  • Shares educational information with staff from workshops and/or journals.
  • Acts as a resource person for nursing personnel.
