About The Position

Come join the family of our State of the Art Long Term Care Nursing Facility! This position requires an understanding of the integumentary system and a willingness to continue to learn. The Skin Integrity Nurse has a current RN license for the state in which he/she is working and is flexible with her/his working schedule.

Requirements

  • Current RN license for the state in which he/she is working.
  • Graduate of an accredited school of nursing with a current licensure as a Registered Nurse in the state where the facility is located.
  • Understanding of the integumentary system.
  • Willingness to continue to learn.

Responsibilities

  • Complete weekly head to toe skin assessments and document in the EHR.
  • Ensure that appropriate treatments and/or interventions are in place for skin issues identified during the skin rounds.
  • Collaborate with the Clinician or Wound NP to obtain new orders or discontinue orders as needed.
  • Update the ADON and DON if any new or worsened ulcers are identified.
  • Update the ADON and DON with any non-healing wounds.
  • Verify/update resident care plans and update documentation for identified skin impairments.
  • Monitor skin integrity practices and procedures to ensure that personnel are implementing the facility’s SIT program.
  • Demonstrate proper infection control practices.
  • Assist in evaluating and implementing recommendations from the Quarterly QA meeting.
  • Ensure that residents with known non-compliance behaviors or are at high risk for pressure ulcers are provided with interventions as appropriate.
  • Attend and participate in weekly SIT meetings.
  • Ensure residents’ responsible parties are updated on wound progress.
  • Proactively develops positive employee relations and promotes teamwork, mutual respect and effective communication.
  • Education of new and inexperienced nurse aides in proper methods of patient care.
  • Performs other duties as requested by the DON or ADON.
  • Possess and demonstrate a high degree of leadership, organizational ability and communication skills.
  • Utilize excellent communications skills to relate to patients, families, and staff members.
  • Constantly update his/her knowledge through ongoing education.
