About The Position

This unique RN position offers the opportunity to build meaningful, long term relationships with residents—beginning in Independent/Residential Living and continuing as they transition into Assisted Living. Approximately 60% of this role supports Residential Living wellness services, with the remaining 40% dedicated to Assisted Living clinical care. In Residential Living, the RN promotes whole person wellness by monitoring residents’ physical and emotional health, providing education and consultation, coordinating preventative screenings, connecting residents with community resources, and serving as a trusted clinical presence during health office hours. The RN must remain attentive to changes in functional status and collaborate closely with the Transitions team. In Assisted Living, the RN is an essential member of the nursing team, ensuring high quality care through ongoing assessment, staff support, care planning, physician communication, and daily resident evaluation. At times, the RN provides direct resident care in accordance with nursing standards, organizational procedures, and State and Federal assisted living regulations. This is an hourly role, with a typical expected schedule Monday -Friday from 8:00am -4:30pm.

Requirements

  • Graduate of an approved school of nursing and current Minnesota RN license.
  • 2–3 years of nursing experience required.
  • Ability to communicate effectively with residents and healthcare professionals.
  • Strong organizational and problem solving skills.
  • Knowledge of medical terminology and understand HIPPA laws.
  • Knowledge of privacy regulations and maintaining patient confidentiality.

Nice To Haves

  • Preferred experience in Case Management, Assisted Living, or Home Health Care.
  • Experience supporting transitions between levels of care is highly desirable.

Responsibilities

  • Participate in resident review as part of the Health & Safety conversation for prospective residents.
  • Support new resident orientation, including emergency system education.
  • Coordinate the Residential Health Office during designated hours, providing medication guidance, answering health questions, and communicating with families as appropriate.
  • Communicate with providers to obtain orders, support transitions in level of care, and facilitate therapy referrals.
  • Serve as first response for Independent Living Call for Aide alerts during office hours; evaluate, document, and follow up on accidents and injuries.
  • Coordinate and implement wellness programming including educational sessions, vaccination clinics, podiatry clinics, and other health‑promotion events.
  • Facilitate weekly Wellness Clinic services such as blood pressure checks and additional offerings based on resident needs.
  • Ensure quality care delivery through assessment, staff monitoring, physician communication, care planning, and daily resident evaluation.
  • Observe and evaluate resident conditions—physical, mental, and emotional—and report significant changes promptly.
  • Provide direct resident care in accordance with nursing standards, procedures, and State/Federal regulations.
  • Provide routine direction and support to staff by explaining nursing procedures, reinforcing training, and ensuring understanding as well as high standards of care and task completion.
