About The Position

The Sub-Acute Nurse is responsible and accountable for coordinating, implementing, and evaluating patient care throughout their shift, with a strong emphasis on collaboration with interdisciplinary teams. You will utilize the nursing process to assess patients, develop and adjust treatment plans, and evaluate outcomes within assigned clinical areas. This role ensures all care meets regulatory documentation standards while actively engaging in multidisciplinary teamwork, Quality Assurance, and Utilization Review activities. As the Sub-Acute Nurse you will also serve as clinical role model, fostering teamwork and providing guidance and consultation to staff within their area of responsibility. The Aspire STRTP (Short Term Residential Therapeutic Program) is a highly individualized residential program that provides relational, trauma-informed therapeutic support and services to foster youth with complex placement and behavioral health needs. Youth receive comprehensive and intensive mental health services in a nurturing home environment for up to 6 months from a multidisciplinary team before transitioning to a sustainable alternative placement. Requirements

  • Bachelor's degree OR Associate Degree in Nursing (ADN) + 2 years of mental health experience
  • Valid California Registered Nurse License
  • Current CPR-BLS certification
  • At least 21 years of age
  • Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy
  • TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements

Nice To Haves

  • Experience with pediatrics, mental health, and/or triage assessment
  • Psychiatric Nursing experience

Responsibilities

  • Evaluate illness or injuries
  • Monitor Physician orders for medication or treatment, and prescriptions filled at outside pharmacies
  • Maintain working relationships with Pediatricians, Specialists, Psychiatrists, Therapists, and hospitals who provide care to clients
  • Arrange appointments or receive orders for treatments when nursing expertise is required to facilitate the process
  • Ongoing training with staff on medications and handling medications
  • Provide other health-related education to staff and clients
  • Document all nursing interventions, including maintaining medication assistance records
  • Collect and analyze assessment data to identify treatment problems and formulate individualized outcomes and plans. Implement and evaluate nursing interventions
  • Review all medical incident reports for shifts
  • Participate in multidisciplinary team meetings
  • Meet regularly with the Program Director and Psychiatrists as requested
  • Collaborate with Psychiatrist to ensure quality of medication administration, adequate consents, medical treatment and follow-up
  • Collaborate with Clinical Manager to ensure health and safety, infection control, resource utilization, and clinical and rehabilitative standards of care
  • Attend appropriate treatment reviews related to health care for youth
  • Provide on-call support for nursing questions, as needed
