About The Position

The Nurse Referral Specialist provides a collaborative approach with nursing, pharmacy, sales and the Revenue Cycle Management department to ensure timely onboarding and ongoing reimbursement of services for all specialty care patients throughout the duration of therapy. This is a remote role, however you must be able to work 8-5:30 Eastern Time Zone hours.

Requirements

  • Current and unrestricted LPN or RN licensure with the ability to maintain all required state licensures for the assigned region.
  • 3–5 years of clinical nursing experience and at least 2 years in infusion therapy, pain management, case management, or intake coordination.
  • Proven ability to manage patient referral and intake processes.
  • Strong communication, collaboration, able to be adaptable, and organizational skills, with the ability to prioritize tasks and meet critical timelines.
  • Thorough understanding of HIPAA and patient privacy regulations in clinical care settings.
  • Proficiency in Microsoft Office and electronic health record/documentation systems.

Nice To Haves

  • Experience conducting patient financial discussions, including coverage verification and out-of pocket responsibilities is preferred.
  • Experience with implanted pump therapy, including catheter management and therapy-related troubleshooting, and/or collaborating with pump manufacturers or specialty pharmacies is preferred.

Responsibilities

  • Oversee complete onboarding of specialty infusion patients.
  • Collect, verify, and document all required intake materials and clinical documentation to ensure timely service initiation.
  • Maintain accurate, timely, and complete documentation in accordance with Pentec Health policies, accreditation standards, and regulatory requirements.
  • Conduct comprehensive financial discussions with patients regarding benefits, out-of-pocket responsibilities, and financial assistance options.
  • Coordinate with Intake, pharmacy, nursing, and reimbursement teams to ensure smooth financial workflow and timely service initiation.
  • Serve as a clinical and logistical resource for patients and caregivers, providing education, reassurance, and ongoing support throughout the referral and onboarding process.
  • Identify and escalate complex care needs or barriers to appropriate internal teams.
  • Facilitate and participate in multidisciplinary case review meetings to resolve clinical, financial, or site-of-service barriers.
  • Uphold HIPAA guidelines and patient privacy policies during all verbal, written, and electronic communications.
  • Attend training, competency assessments, and educational in-services as required to maintain clinical knowledge and licensure.
  • Participate in quality improvement initiatives to streamline onboarding processes and enhance the patient experience.
  • Perform other duties and special projects as assigned, demonstrating flexibility and proactive problem-solving.
