About The Position

Care2U is a fast-growing healthcare startup providing on-demand, in-home urgent and acute care throughout Manhattan, the surrounding boroughs, Westchester County, and Nassau County. The organization focuses on patient-centered care and aims to keep patients at home and out of the hospital. Nurse Practitioners and Physician Assistants respond to patient homes and treat patients in collaboration with Telehealth Emergency Physicians. The role includes conducting in-person home visits to manage acute illnesses and injuries; providing evidence-based treatment and clinical management; and working with field providers and care coordinators to manage complex care needs at home. Providers assess risk factors affecting acutely ill and complex chronically ill patients, including medication compliance and side effects, limited mobility, impaired nutrition, fall safety, and social isolation. They maintain a safe, secure, equitable, and healthy work environment, protect patient confidentiality, and complete timely Electronic Health Record documentation that meets acute-care compliance standards and captures patient acuity. They respond to on-demand and scheduled calls, focus on quality and efficiency metrics, and stay current on in-home acute care literature, technology, and regulatory issues. The position requires a valid New York medical license, NP or PA certification and licensure, credentialing qualifications including a DEA certificate and BLS certification, and experience in emergency department or hospitalist medicine. A Master’s Degree in PA or NP Studies from an accredited university is required. Additional experience in geriatric medicine, hospital-at-home, or post-acute care is a plus. This is a full-time, in-person role with traditional 8-, 10-, and 12-hour shifts and on-demand responses. The posting also asks whether candidates can work at least four shifts or 48 clinical hours per month.
