About The Position
Stafford Services is redefining clinical care for residents with complex needs in skilled nursing facilities (SNFs). They are seeking a highly motivated Board-Certified Nurse Practitioner to lead care within multiple facilities. The role involves providing expert routine and acute medical care directly to SNF residents, conducting thorough assessments, managing complex chronic conditions, overseeing personalized care plans, and partnering with facility nursing leadership and attending physicians to ensure integrated care. The Nurse Practitioner will also actively support quality improvement initiatives, focusing on reducing preventable hospital readmissions.
Requirements
- Active and unrestricted Nurse Practitioner License in the State of Ohio.
- Active Nurse Practitioner Certification.
- 3+ years of prior RN experience is preferred for this specialized setting (for new graduates).
Nice To Haves
- Skilled Nursing/Rehab experience is preferred.
- Highly motivated new graduates will be considered.
