About The Position

LifeLong Medical Care is seeking a Family Nurse Practitioner to provide high-quality outpatient primary care at their Downtown Oakland Health Center. The role involves working within a collaborative team model that integrates case management and behavioral health services to address patients' medical and psychosocial needs. The health center utilizes a multidisciplinary team approach, including primary care providers, RNs, Social Workers, Psychiatry, and Podiatry. This position offers the opportunity to provide comprehensive care to underserved populations and make a significant impact on patients' lives.

Requirements

  • Master’s or Doctorate degree from an accredited Nurse Practitioner program.
  • Current California Registered Nurse (RN) license and Nurse Practitioner license.
  • National certification as an NP in Family, Adult-Gerontology, or a relevant specialty.
  • Furnishing license and DEA certification required or obtainable upon hire.
  • Current Basic Life Support (BLS) certification.
  • Demonstrated commitment to providing care to underserved communities and vulnerable populations.
  • Excellent clinical, organizational, and communication skills.
  • Ability to work both independently and as part of a collaborative, team-based model.
  • Cultural humility and sensitivity in working with diverse populations.

Nice To Haves

  • Minimum 1–2 years of clinical experience preferred (new grads may be considered).
  • Experience in a Federally Qualified Health Center (FQHC) or community clinic setting.
  • Proficiency in Epic EHR system.
  • Bilingual or multilingual skills.

Responsibilities

  • Perform physical examinations, obtain medical histories, diagnose illnesses, develop treatment plans, and prescribe medications and therapies in accordance with standardized procedures and applicable scope of practice.
  • Provide preventive and primary care services, including immunizations, health screenings, and wellness counseling.
  • Manage chronic conditions such as diabetes, hypertension, asthma, and depression, with a strong focus on patient education and self-management support.
  • Document all clinical encounters thoroughly and accurately in the Electronic Health Record (EHR) system.
  • Collaborate with an interdisciplinary care team to address medical, behavioral, and social determinants of health.
  • Provide acute care visits and triage support as needed.
  • Support transitions of care, referrals, and follow-up for diagnostic tests and specialty care.
  • Participate in clinical huddles, staff meetings, and quality improvement initiatives.
  • Maintain awareness of and adherence to clinical protocols, policies, and regulatory requirements.
  • Serve as a preceptor for students or trainees when appropriate.
  • Perform other duties as assigned.
