About The Position

You will lead a sustainable patient panel with full support from our interdisciplinary care team and exercise true autonomy to solve problems and do what is right for the patient.

Requirements

Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
Active, unencumbered NP license and Board Certification (ANCC or AANP) in Wisconsin.
Valid driver’s license and vehicle for local field-based travel.
Must be legally authorized to work in the United States without the need for employer sponsorship.
Must successfully complete a pre-employment drug screening.
Must successfully complete a background check.
Must have satisfactory results on monthly FACIS checks.

Responsibilities

Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home.
Create longitudinal care plans that move the needle for high-risk patients.
Partner with an interdisciplinary team to ensure seamless, safe care transitions.
Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.
