About The Position

360care is seeking a Nurse Practitioner to provide foot-care services in a skilled nursing environment. The role focuses on delivering quality patient care to residents of senior care communities. Clinicians make clinical decisions and work independently as well as collaboratively with on-site staff. Patients are seen on-site, either in a treatment room or room to room within the facility. The position involves working with the geriatric population and traveling to facilities. 360care provides equipment and supplies, on-the-job training, scheduling and billing, a laptop for documentation, office support from a Care Coordination team, and a collaborative physician for nurse practitioners. The Care Coordination team handles consents, pre-authorizations, patient scheduling, post-operative support, and IT support.

Requirements

Valid, unrestricted state licensure.
Active, unrestricted NP/FNP license.
Valid driver’s license and willingness to travel to facilities.
Desire to work with the geriatric population.
Ability to work both independently and collaboratively with on-site staff.

Nice To Haves

Experience in a podiatry office, wound care, or orthopedics is a plus.
Foot-care experience.

Responsibilities

Provide routine foot and nail care, including trimming nails and reducing thick nails.
Provide vascular and diabetic foot care.
Conduct fall-risk assessments and implement interventions.
Perform minor foot and nail care procedures.
Work independently and collaboratively with on-site staff.
Travel to facilities to provide services.
