About The Position

Community Care is hiring a Nurse Practitioner for our Pace Program. This is a part-time opportunity at our Racine/Kenosha location.

Requirements

• Masters degree in Nursing required.
• Wisconsin Registered Nurse/RN license required.
• Family, Geriatric, or Adult NP certification is required.
• DEA registration required.
• Advance Nurse Prescriber required.
• NPI and CPR & AED required
• WI Driver License
• minimum of 1 year recent clinical nursing experience with an at risk or elderly population.

Responsibilities

• Documents and updates health history and findings, physical exam, functional status evaluations, diagnostic test and lab evaluations, mental health status and function evaluations, preventative care, and health reminders.
• Within their official scope and practice, delegates appropriate tasks to interdisciplinary team staff, and provides leadership and guidance. Acts as a resource to staff to ensure appropriate care and follow through and provides care planning assistance.
• Educates members and families regarding health maintenance and acute and chronic illnesses.
• Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
