About The Position

The Clinical Programs Nurse Practitioner (NP) for the Cut Hypertension Program provides advanced clinical care, including independent diagnosis, treatment, and medication management for adults with hypertension in community-based barbershop and salon settings. As a primary clinical lead on-site, the NP utilizes advanced clinical judgment to implement hypertension protocols, manage complex patient needs, and provide culturally grounded, patient-centered care. Roots Community Health is recruiting a Nurse Practitioner to support the Cut Hypertension Program in Sacramento. This long-standing initiative, launched in 2010, is grounded in evidence that African American barbershops and salons serve as trusted settings for health prevention, community engagement, and outreach. The program addresses long-standing disparities in cardiovascular disease among the African American community by offering accessible and culturally grounded hypertension screening, treatment, and care coordination in partnership with barbers, cosmetologists, community leaders, and clinical teams. The Nurse Practitioner will play a central role in delivering high-quality clinical care in a community environment and strengthening continuity between barbershop/salon-based services and primary care. Position Summary: NP provides advanced clinical care, including independent diagnosis, treatment, and medication management for adults with hypertension in community-based barbershop and salon settings. As a primary clinical lead on-site, the NP utilizes advanced clinical judgment to manage complex patient needs and provide culturally grounded, patient-centered care. The NP collaborates with pharmacists, community health workers, and the broader Roots Community Health team to ensure consistent service delivery.

Requirements

  • Current and unrestricted Nurse Practitioner license in the State of California.
  • Valid Nurse Practitioner Furnishing Number and DEA registration.
  • Clinical experience in primary care, chronic disease management, community health, or related settings.
  • Ability to work effectively and maintain professionalism in non-traditional, community-based clinical settings.
  • Ability to attend weekly virtual team meetings and in-person meetings as applicable.

Nice To Haves

  • Experience collaborating with barbers, faith leaders, or community partners is helpful.

Responsibilities

  • Provide complete subjective and objective assessments during barbershop/salon visits, including evaluation of symptoms, medication use, lifestyle factors, and home blood pressure data.
  • Perform accurate blood pressure measurement using automated office and manual blood pressure techniques.
  • Independently diagnose and manage hypertension and related comorbidities within the program scope.
  • Utilize personal DEA and Furnishing numbers to e-prescribe medications and manage pharmacologic therapy.
  • Order laboratory tests required for medication management and review results to identify electrolyte abnormalities or changes in kidney function.
  • Determine blood pressure control relative to target goals and establish individualized treatment plans and follow-up schedules.
  • Identify and provide initial management or prompt escalation for clinical findings such as hypertensive urgency or symptomatic hypotension. Provide clear and culturally responsive education on lifestyle interventions including nutrition, sodium intake, physical activity, and tobacco cessation.
  • Serve as the supervising provider for community health worker services and offer real-time clinical guidance and workflow support to barbers and program staff.
  • Use the electronic medical record and mobile platform to document all assessments, care decisions, medication adjustments, and laboratory orders.
  • Coordinate with primary care providers to support continuity of care between barbershop-based services and clinic-based services.
  • Maintain professional boundaries and uphold privacy and safety while delivering care in informal community environments.
  • Participate in program improvement efforts focused on strengthening care processes and service quality.
