About The Position

Humana's Primary Care Organization is seeking a full-time Primary Care Nurse Practitioner to join a team redefining senior primary care. This role is within a fast-growing provider of senior-focused, value-based care, with centers across 15 states under the CenterWell and Conviva brands. Requirements

• Graduate of an accredited Nurse Practitioner program (MSN or DNP)
• Current national certification (AANP or ANCC)
• Active, unrestricted NP license in SC
• DEA license and Medicare/Medicaid credentials (or ability to obtain upon hire)

Nice To Haves

• 2+ years Nurse Practitioner experience
• Experience in primary care, or Medicare Advantage settings with geriatric patients
• Experience with Athena EMR
• Familiarity with HEDIS quality indicators and Medicare guidelines

Responsibilities

• Report to the Associate Medical Director
• Work fully onsite in Homeland Park, SC
• Provide comprehensive outpatient care for senior patients
• Maintain accurate documentation and medical records
• Diagnose and manage moderately complex conditions
• Coordinate referrals and collaborate with specialists
• Partner with interdisciplinary teams for holistic care
