This position, in coordination with the supervising physician, is primarily responsible for increasing access to substance abuse and medication assisted treatments; coordinating and managing health care services and health education; encouraging health and wellness and disease prevention. The candidate will effectively engage patients; will report to the Medical Doctor for clinical supervision and the Chief Program Officer/Vice President of Behavioral Health Services for administrative supervision. Requirements

  • NYS License to practice as a Registered Professional Nurse and certification by the NYSED in medicine to practice as a Nurse Practitioner.
  • Must be eligible and maintain eligibility for full and unconditional participation in the Medicaid and Medicare programs.
  • DATA 2000 “X” waiver.
  • NYS Prescriptive Rights
  • Behavioral Health background preferred.
  • Must have computer skills.
  • Must successfully complete a criminal background check, obtain clearance from the Office of Children and Family Services and be drug free.

Nice To Haves

  • bilingual Spanish/English skills are a plus.

Responsibilities

  • Provide direct patient care, including assessment, monitoring, and treatment of patients with medical, mental health, and substance use disorders.
  • Deliver addiction medicine services, including Buprenorphine inductions/maintenance, IM Naltrexone treatment, medication management, and emergency medical care when needed.
  • Conduct HIV and Hepatitis C screenings/testing and coordinate referrals for treatment and specialized medical or behavioral health services.
  • Collaborate closely with physicians, behavioral health staff, care managers, peers, and multidisciplinary teams to ensure continuity and coordination of care.
  • Maintain accurate and timely documentation, treatment plans, medication orders, and EHR records while adhering to all agency and funding source policies and procedures.
  • Work within non-traditional community-based settings, including mobile units serving underserved populations, and participate in staff meetings, trainings, and occasional evening/weekend coverage.
  • Coordination of care with Behavioral Health Team (CASAC, Mental Health clinicians, Care Manager, Peers) to ensure continuity of care.
  • Provide consultation and training to on-site staff.
  • Assist with the identification and counseling of mental health and substance abuse problems.
  • Work in a non-traditional environment; e.g. – mobile unit visiting underserved, disadvantaged and disenfranchised communities and populations.
  • Participates in departmental staff meetings and committees as required.
  • Is an integral part of the treatment team, consulting with clinicians and participating in team meetings and case conferences; resolving participant problems utilizing multidisciplinary team strategies.
