About The Position

Integrated Wound Care is seeking a compassionate and skilled Nurse Practitioner for a part-time opportunity focused on wound care. The company develops and provides wound care services to skilled nursing, long-term care, and rehabilitation facilities. Its programs aim to exceed clinical standards for wound healing while improving efficiency and reducing wound-related hospitalizations. In this role, the Nurse Practitioner will contribute to patient outcomes by assessing wounds, creating individualized treatment plans, collaborating with healthcare professionals, documenting care, educating patients and families, and monitoring progress.

Requirements

Active license in the state of employment.
At least 1 year of wound care experience.
Ability to work independently and as part of a team.
Exceptional critical-thinking and decision-making skills.
Excellent verbal and written communication skills.
Strong time-management and organizational abilities.
Comfort with technology and electronic medical records.

Responsibilities

Conduct wound assessments and provide personalized treatment plans for patients.
Work collaboratively with a team of healthcare professionals to ensure optimal patient care.
Complete documentation accurately and in a timely manner.
Educate patients and their families on wound care, prevention, and management.
Monitor patient progress and adjust treatment plans accordingly.
