About The Position

First Care Clinic is seeking applications for a Full-Time Nurse Practitioner. The Nurse Practitioner is responsible for providing medical care for patients in collaboration with other FCC physicians within the scope of his/her practice.

Requirements

• Current Advanced Practice Registered Nurse (APRN) licensure in the State of Kansas
• Active Drug Enforcement Administration (DEA) certificate

Responsibilities

• Provide medical care for patients of all ages including an assessment, interpretation of data, and development and implementation of a therapeutic plan.
• Place a special emphasis on preventative care.
• Ability to determine when patients require care that is outside of scope and collaborate with other FCC physicians and outside providers as necessary.
• Follow-up with patients regarding lab results or other medical care concerns.
• Provide care to nursing home patients as assigned.
• Meet regularly with the Medical Director to coordinate health care plans and review/evaluate the collaboration efforts.
• Be aware of existing agencies and resources in the community for patient referrals.
• Assist and participate in the work of community health projects and educational programs.
