PRN Nurse Practitioner (Hospice)

We’re hiring a Nurse Practitioner – a compassionate and skilled clinician who provides advanced nursing care, supports patients and families through end-of-life journeys, and ensures compliance with regulatory standards. Requirements
- Graduate of an accredited school of Advanced Practice Registered Nursing
- MSN or Doctoral Degree required
- Current Advanced Practice Registered Nurse License in the state of employment
- End-of-Life Nursing Education Program (ELNEC) – all 8 modules completed within 18 months of hire
- Valid driver’s license, good driving record, and reliable transportation
- Ability to lift up to 50 pounds
- Must clear criminal background check, physical, and drug screening
- Comfortable working outside regular hours as needed
Nice To Haves
- Certification in Hospice and Palliative Nursing preferred
Responsibilities
- Perform comprehensive nursing assessments for hospice patients
- Assess signs and symptoms of discomfort, including pain, and take prompt, appropriate action
- Initiate and manage individualized plans of care for patients and families
- Provide direct face-to-face visits for referred and existing patients
- Communicate with physicians regarding patient updates and care plans
- Maintain knowledge of state and federal regulations and JCAHO standards (if applicable)
- Participate in Quality Assessment and Performance Improvement (QAPI) programs
- Ensure all services comply with hospice standards and regulatory requirements
