About The Position

Hueman is hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Newark, Delaware. This flexible opportunity is designed to accommodate individual scheduling preferences and may provide supplemental income and connection with the community. Nurse Practitioners meet with members in their homes, where they can interact in a more personal setting than a typical clinic visit. Practitioners performing HRAs are not required to treat patients or prescribe medication in the home. The role includes travel to patients’ homes, comprehensive non-invasive physical assessments, medical-history review, documentation, medical reconciliation, and basic patient education about wellness and preventive healthcare. Requirements

Master’s degree, or commensurate experience and satisfactory completion of NP license.
Active state Nurse Practitioner license.
Active national board certification.
Current BLS certification.

Nice To Haves

12 months of experience as a paid advanced clinical provider is preferred but not required.

Responsibilities

Travel to and from patients’ homes.
Perform comprehensive, non-invasive physical assessments.
Review medical history and ensure accurate and timely documentation.
Conduct medical reconciliation to ensure patient safety and compliance.
