About The Position

Physician Solutions Group is accepting candidates for a full-time Certified Registered Nurse Practitioner (CRNP) position in the Bethlehem area. As a member of the clinical team, the Nurse Practitioner applies advanced knowledge and a diverse skill set to assess the healthcare needs of chronically ill geriatric members, working in collaboration with physicians and interdisciplinary teams. The role focuses on helping members maximize their quality of life and live as independently as possible in their homes and communities. The position follows a Monday–Friday daylight schedule, with no evenings, weekends, or holidays. The role includes conducting assessments and examinations, developing and reviewing care plans, coordinating services, performing procedures, and educating staff, members, caregivers, and families.

Requirements

Be a certified registered nurse practitioner.
Hold a valid Pennsylvania nursing license.
Be willing to participate in continuing education and workshops to maintain a valid license.
Have at least one year of experience working with an elderly community.
Have a strong passion for helping the geriatric population.

Responsibilities

Perform health histories and in-depth medical examinations for new members to determine preliminary Plans of Care.
Initiate, evaluate, and review medical orders, and identify opportunities to improve care when possible.
Carry out procedures and document care and recommendations.
Order, interpret, and follow up on laboratory tests and results.
Request consultations from in-house and external medical and non-medical providers, including physicians, therapists, dietitians, social workers, and other specialists.
Educate staff, members, caregivers, and families on essential care functions to help members maintain maximum independence at home and in centers.
Complete ongoing assessments and determine Plan of Care changes or improvements based on member feedback and medical evaluations.
