About The Position

The Nurse Practitioner records physical findings and formulates a plan and prognosis based on the patient's condition. The role collaborates with physicians and other health professionals to prepare comprehensive patient care plans, prescribes or recommends drugs or other treatments, and monitors patient progress, reporting updates to the supervising physician. The Nurse Practitioner works collaboratively with hospice employees and volunteers as part of an interdisciplinary team (IDT).

Requirements

• At least two years of experience in hospice, home health, or community health within the past five years.
• Completion of an accredited Nurse Practitioner program.
• State licensure is required.
• Ability to lift objects in excess of 20 pounds.
• Ability to stand or walk for more than one hour per day and to talk, hear, sit, stoop, kneel, and crouch as needed.
• Close vision, distance vision, and ability to adjust focus.

Responsibilities

• Discuss cases with physicians and other health professionals to prepare comprehensive patient care plans.
• Prescribe or recommend drugs or other forms of treatment.
• Monitor and maintain patient progress and report progress to the supervising physician.
• Collaborate with hospice employees and volunteers as part of an IDT.
• Assist with lifting and transferring patients as required.
