About The Position

The Family Nurse Practitioner will provide personalized, proactive, patient-centered care focused on wellness, early intervention, chronic disease management, and long-term health. Care is delivered in a collaborative clinic environment that values quality over volume and includes both in-person and virtual visits.

Requirements

• Active and unrestricted FNP license in the practicing state.
• Board certification as an FNP.
• DEA registration.
• Experience treating patients ages 2 and older.
• Familiarity with HIPAA and a strong commitment to patient confidentiality.
• Ability to pass on-site credentialing.

Nice To Haves

• Two or more years of primary care experience (preferred).

Responsibilities

• Provide comprehensive care through in-person and virtual visits to increase access and continuity of care.
• Prescribe medications and develop personalized health plans with patients.
• Educate patients on wellness, prevention, and lifestyle management.
• Collaborate with physicians, medical assistants, and other clinicians.
• Contribute to a positive, team-oriented clinic culture.
