This is a full-time outpatient opportunity within a value-based care model focused on adult and geriatric populations, emphasizing prevention, outcomes, and long-term patient relationships. POSITION OVERVIEW The Nurse Practitioner delivers comprehensive primary care in a collaborative, team-based environment. Providers manage their own patient panels while working alongside physicians, care managers, and clinical support staff to deliver coordinated, high-quality care. Requirements

  • Master’s Degree in Nursing or equivalent NP program required
  • National certification required (FNP, AGPCNP, ANP, or GNP)
  • Active, unrestricted Nurse Practitioner license required
  • Active DEA license required
  • U.S. work authorization required

Nice To Haves

  • Experience in primary care, geriatrics, or internal medicine preferred
  • Experience with value-based care models preferred
  • Knowledge of HCC coding, ICD-10, and health risk assessments preferred
