About The Position

The Nurse Practitioner or Physician Assistant is responsible for delivering comprehensive primary care to a dedicated panel of geriatric patients. This includes visiting assisted and independent living facilities (ALFs / ILFs), private homes (house calls), and other group settings (apartment buildings, senior housing complexes, etc.).

The new provider will see medically complex, frail, and geriatric adults who may have:
- high clinical complexity
- multi-morbidity
- functional decline
- mobility limitations
- polypharmacy
- cognitive impairment
- caregiver burden
- increased risk for avoidable utilization
- evolving goals of care

This requires providers to build and maintain therapeutic relationships with patients and caregivers, including providing emotional support and education as needed. The practice uses the AthenaCare EHR.

Requirements

• North Carolina NP or PA license
• DEA
• 1+ year advanced practice experience in a relevant environment

Nice To Haves

• previous home health or long-term care facility (as RN or APP) preferred
