This is a full-time outpatient opportunity within a value-based care model focused on delivering high-quality, patient-centered outcomes for adult and geriatric populations. POSITION OVERVIEW The Primary Care Physician provides comprehensive outpatient care within a team-based, value-driven environment. This model allows physicians to focus on quality of care, prevention, and long-term patient relationships rather than high patient volume. Requirements

  • MD or DO required
  • Board Certification or Board Eligibility in Internal Medicine or Family Medicine required
  • Active, unrestricted state medical license required
  • Active DEA license required
  • U.S. work authorization required
  • Experience in outpatient primary care preferred
  • Experience with geriatric populations preferred
  • Value-based care experience preferred but not required
  • Strong collaboration and team-based care mindset
  • Ability to manage patient panels and drive outcomes
