This is a full-time outpatient opportunity within a value-based care model focused on delivering high-quality, patient-centered outcomes for adult and geriatric populations. POSITION OVERVIEW The Primary Care Physician provides comprehensive outpatient care within a team-based, value-driven environment. This model allows physicians to focus on quality of care, prevention, and long-term patient relationships rather than high patient volume. Requirements

MD or DO required
Board Certification or Board Eligibility in Internal Medicine or Family Medicine required
Active, unrestricted state medical license required
Active DEA license required
U.S. work authorization required
Experience in outpatient primary care preferred
Experience with geriatric populations preferred
Value-based care experience preferred but not required
Strong collaboration and team-based care mindset
Ability to manage patient panels and drive outcomes
