This is a value-based care model focused on quality outcomes, preventive medicine, and delivering an exceptional patient experience to Medicare populations. POSITION OVERVIEW The Primary Care Physician provides comprehensive outpatient care within a team-based, value-driven environment. Physicians are supported by dedicated care teams, allowing focus on patient outcomes rather than volume-based metrics. Requirements

  • MD or DO required
  • Board Certified or Board Eligible in Internal Medicine or Family Medicine
  • Active, unrestricted state medical license required
  • Active DEA license required
  • US work authorization required
  • Ability to work in a collaborative, team-based environment

Nice To Haves

  • Experience in geriatric care preferred
  • Experience in value-based care preferred
  • Outpatient primary care experience preferred
  • Bilingual skills a plus (not required)
