About The Position

Tembo Health is a digital health company focused on improving care for older adults, particularly in dementia care management through services like the CMS GUIDE program and virtual urgent care. As the first full-time nurse practitioner, you will lead patient and family engagement in the dementia diagnosis journey, initiating diagnostic evaluations and supporting the care navigation team with medical escalations. You will collaborate with Tembo's Head of Medical to refine diagnostic workflows and develop a new specialty care model for dementia patients.

Requirements

  • Family or adult-gerontology nurse practitioner with 1 year minimum experience.
  • Licensed in Arizona, Oregon or Nevada with the ability to be licensed to practice virtual/outpatient care in any state.
  • Must not be excluded from participating in any state or federal healthcare programs.
  • Passion for caring for patients with dementia.
  • Comfort with learning new technology systems.

  • You are comfortable working from home embedded in an all-remote team.
  • You’re a self-starter who identifies problems and proposes solutions, but also likes to work on an interdisciplinary team and knows when to escalate when you need help.

Responsibilities

  • Conduct virtual assessments of patients with suspected dementia diagnoses, including baseline cognitive assessments and further laboratory testing orders as needed.
  • Conduct medication reviews of patients with dementia, supporting patients and their medical care team with recommendations to reduce high-risk medications and to start medications that may help their symptoms where indicated.
  • Develop comprehensive care plans for patients in collaboration with care navigators.
  • Coordinate care alongside care navigators and patient’s primary care teams including external care providers, specialists, etc. to support the patient and their caregiver(s).
  • Support other clinical needs that arise, including urgent care or other geriatric practice needs as the practice evolves.
