About The Position

Community Care is hiring a Nurse Practitioner for its PACE Program. The role supports elderly individuals and adults with physical and intellectual disabilities in living as independently as possible in the community. This is a full-time position serving the Racine/Kenosha and Milwaukee locations, with regular local travel using a personal vehicle. The schedule is Monday through Friday, 40 hours per week, from 8:00 AM to 4:30 PM. On-call responsibilities are required 6–7 weeks per year. A $10,000 sign-on bonus is offered. Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live independently in the community. It coordinates and delivers supportive services to more than 13,000 Wisconsin residents and has local offices supporting the 23 counties it serves.

Requirements

Master’s degree in Nursing.
Wisconsin Registered Nurse (RN) license.
Family, Geriatric, or Adult Nurse Practitioner certification.
DEA registration.
Advanced Nurse Prescriber credential.
NPI and CPR and AED credentials.
Wisconsin driver’s license.
At least 1 year of recent clinical nursing experience with an at-risk or elderly population.
Personal vehicle and proof of required insurance coverage for regular local travel.

Responsibilities

Document and update health histories and findings, physical examinations, functional status evaluations, diagnostic tests and laboratory evaluations, mental health status and function evaluations, preventative care, and health reminders.
Within the official scope of practice, delegate appropriate tasks to interdisciplinary team staff and provide leadership and guidance.
Serve as a resource to staff to ensure appropriate care and follow-through, and provide care-planning assistance.
Educate members and families about health maintenance and acute and chronic illnesses.
Travel locally on a regular basis using a personal vehicle.
