About The Position

The Mobile Wellness Clinic midlevel provider is a licensed nurse practitioner (N.P.) or licensed physician assistant (P.A.) responsible for the provision of primary care and urgent/episodic care to the clinic's patients. The P.A. or N.P. provides a full scope of ambulatory care services largely geared to underserved populations. This care consists of comprehensive health screenings, establishing medical diagnosis for acute and chronic health problems, evaluation of normal and abnormal findings, initiation of treatments and referral services. These functions will be performed in accordance with standardized Procedures/Process Oriented Protocols approved by the Dominican Hospital Credential/Inter Disciplinary Practice Committee. The midlevel provider will educate the patient and/or family about preventive care, medical issues, diagnosis and use of prescribed medical treatments and medications. The provider will maintain legible, accurate and confidential medical records in compliance with state regulatory standards and clinic policies and procedures. The midlevel provider is part of a multidisciplinary practice that offers a blend of individual and population-based care.

Requirements

  • Licensed nurse practitioner (N.P.) or licensed physician assistant (P.A.).
  • Minimum of three (3) years experience in community health clinic.
  • BLS Basic Life Support from American Heart Association.
  • Registered Nurse Practitioner: CA (RNP:CA).
