About The Position

Tower Health at Home is seeking a full-time Nurse Practitioner to join their team. The Nurse Practitioner will address the day-to-day clinical needs of hospice patients in home and hospital settings, including face-to-face encounters for recertification of hospice benefit periods (virtual or in-person). Responsibilities include managing symptom management needs, addressing patient concerns, completing F2F visits as per regulations, seeing GIP patients, and educating the community/staff about hospice. GIP patients may be located at hospitals in Reading, Pottstown, and Phoenixville. The schedule is weekdays from 8 am to 4 pm, with evening and weekend call coverage split with the Palliative Medicine team for 24/7 coverage of all hospice and palliative medicine patients. An attending will always be on backup call with the NP, and call is performed remotely from home. Tower Health at Home provides a comprehensive range of services allowing patients to recover from illness or manage chronic conditions at home. The team works collaboratively to deliver compassionate, high-quality care in patients' homes for their home health or hospice needs. Requirements

  • Master's Degree
  • Relevant Experience
  • BLS Certification
  • NCCPA Certification
  • PA Physician Assistant License
  • PA DEA License
  • DOT Medical Examiner Certification
  • Dictation and Transcription Skills
  • Excellent Communications Skills
  • Excellent Interpersonal Skills
  • Microsoft Office Applications
  • Completes and maintains all competencies and trainings as required for role.
