About The Position

Circadia Health is a growth-stage healthcare AI company focused on preventing avoidable hospitalizations and improving senior-care operations. Its platform combines contactless sensing, predictive models, and enterprise integrations, and its technology supports post-acute patients across skilled-nursing, home-health, and home-care networks. The on-site Nurse Practitioner provides direct patient care at partner facilities and uses predictive health signals to support earlier interventions and reduce preventable readmissions. The role combines patient assessments and care planning with staff coaching, workflow integration, and feedback to clinical operations and product teams. This is a full-time, in-field position with Monday–Friday presence at partner facilities in Walnut Creek, California. Local travel to sites within a 30–40-mile radius is expected; mileage and lodging are covered per policy for sites outside that radius. Success is measured by patient visits, staff engagement, acknowledgement times, timely documentation, workflow adoption, and trends in falls and readmissions. The anticipated annual base salary is $140,000–$180,000, with placement depending on job-related factors. Circadia Health employs the Nurse Practitioner and bills for the practitioner’s services on their behalf.
