About The Position

The Nurse Practitioner will join a collaborative, patient-centered physiatry team and provide care that supports patients’ functional outcomes and independence. New graduates are encouraged to apply. Requirements

Active license to practice in the state of employment.
Current Controlled Substance License, required upon start date.
Current DEA License, required upon start date.
ANCC Certificate or AANP Certificate.
Certified Nurse Practitioner certification is required.
Desire to work in Physiatry/Physical Medicine and Rehabilitation.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients with diverse rehabilitation needs.
Conduct patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine rehabilitation goals and interventions.
Monitor patient progress and adjust treatment plans as needed to optimize functional outcomes and promote independence.
Participate in multidisciplinary team meetings, case conferences, and educational activities.
Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical record systems.
Educate and counsel patients and their families about the rehabilitation process, expected outcomes, and post-discharge care.
