Requirements

• Professional licensure in Delaware as a Nurse Practitioner
• Clinical competence in the care of adult and geriatric patients
• Open to serving older adults; people who have loss of cognitive functioning: individuals who have experienced traumatic brain injury; those who are experiencing mental illness; people who have substance use disorders; and those experiencing end of life issues.

Responsibilities

• Provide primary care to a case load of approximately forty patients
• Round on units daily and evaluate patients
• Conduct examinations, perform diagnostic tests, provide preventive care, develop and manage treatment plans for acute and chronic diseases, and prescribe medications
• Provide consultation and health education to nursing and ancillary staff
• Collaborate with the facility's onsite Medical Director, other Nurse Practitioners, RNs, and ancillary staff
• Actively participate in multidisciplinary treatment team meetings
