About The Position

A full-time Nurse Practitioner position is now available with Liberty Healthcare’s team at DHCI – a highly rated skilled nursing facility located in Smyrna, Delaware. This role offers the ability to spend ample time with patients without productivity requirements, an opportunity to positively impact the health and wellbeing of older adults and people with disabilities, and a very nice work/life balance.

Requirements

• Must be a licensed Nurse Practitioner

Responsibilities

• Provide primary care to a case load of approximately forty patients
• Round on units daily and evaluate patients
• Conduct examinations, perform diagnostic tests, provide preventive care, develop and manage treatment plans for acute and chronic diseases, and prescribe medications
• Provide consultation and health education to nursing and ancillary staff
• Collaborate with the facility's onsite Medical Director, other Nurse Practitioners, RNs, and ancillary staff
• Actively participate in multidisciplinary treatment team meetings
