Bft Nurse Practitoner Community Care Vans

Mass General BrighamSomerville, MA

About The Position

Bulfinch Temps is hiring a temporary Nurse Practitioner (NP) to cover a maternity leave for the BWH Community Care Vans. This role is temporary and not benefitted. The position requires 24-40 hours per week, with availability for over 20 hours including weekends and some evenings. The NP will be a core clinical member of the mobile team, providing patient-centered services such as cardiometabolic-focused visits, colorectal cancer screening, substance use disorder screening and care, vaccinations, and screening for social determinants of health with connection to resources. The NP will provide direct clinical care during approximately 5-6 mobile clinic sessions per week at multiple community sites, supporting clinic set-up and flow, coordinating with on-site staff, and ensuring timely, high-quality patient service. Due to clinics occurring in community settings without immediate on-site clinical back-up, the NP must practice with a high degree of independence and clinical judgment, with demonstrated ability to anticipate, prevent, and manage complex presentations and urgent/emergent situations for patients across the lifespan. Clinics are often held in the evenings and on weekends, primarily in Boston neighborhoods with occasional clinics in other communities such as Chelsea and Lynn. The NP may also be asked to assist with follow-up calls to patients to support blood pressure management.
