About The Position

BAYADA Hospice is hiring a compassionate, dedicated Per Diem Nurse Practitioner to join its team in the Tucson, Arizona coverage area. The Nurse Practitioner is an integral member of a multidisciplinary health care team, providing palliative care that helps clients receive the medical care they need while remaining at home. Prior home care, hospice, or palliative care experience is strongly preferred but not required.

Requirements

Current Nurse Practitioner license in Arizona.
At least one year of recent, verifiable clinical (medical/surgical) experience.
Solid computer skills.
Ability to travel to assigned cases.
Strong interpersonal skills.

Nice To Haves

Prior home care, hospice, or palliative care experience.
Prior experience with electronic medical records (EMR).

Responsibilities

Make in-home face-to-face hospice recertification visits to clients in designated geographic territories.
Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
Follow up on, execute, and properly document doctors’ orders.
Perform client assessments as necessary.
