About The Position

The University of California, San Francisco (UCSF) Heart & Vascular Center’s Advanced Heart Failure Comprehensive Care Center is seeking a full-time Advanced Practice Provider (APP) to treat patients with mechanical circulatory support (MCS), heart transplant, and advanced heart failure. The UCSF team is comprised of experienced physicians, surgeons, APPs, nurses, pharmacists, social workers, and researchers dedicated to providing comprehensive care across the entire spectrum for patients with advanced heart failure. Our Advanced Heart Failure & MCS programs are recognized nationally for their innovative care and support systems in both outpatient and inpatient settings. The APP will function as the primary provider for these patients in the hospital, collaborating closely with the Advanced Heart Failure Cardiologist, Cardiothoracic Surgery, and Intensive Care teams, as well as nursing, case management, social workers, and other multidisciplinary teams. This role involves evaluating, treating, and managing a highly complex patient population within the Intensive Care and Acute Care Units.

Requirements

Minimum of 4 years as a Nurse Practitioner in MCS, or Advanced Heart Failure
Minimum of one 2 years managing mechanical circulatory support patients
3 years critical care nursing experience managing all MCS devices as a: impella, extracorporeal membrane oxygenation (ECMO), left ventricular assist device (LVAD)

Responsibilities

Pre-rounds on patients daily, providing a medical overview and care management plan to attending physicians.
Works closely with the CTS, CICU and AHF attending MDs, fellows, and hospitalists to manage both primary and consult MCS and AHF patients.
Independently manages hospitalized patients across the AHF, MCS, and other Heart & Vascular service lines, escalating to the attending MD when needed.
Monitors and transports MCS patients through procedures and studies as required.
Manages and coordinates clinical care for Advanced Heart Failure, MCS, Pulmonary Hypertension and Heart Transplant patients in collaboration with the AHF, CTS, and CICU teams.
Assesses and monitors patients during MCS procedures per clinical practice guidelines.
Performs nursing histories and physical exams.
Documents follow-up for VAD patients.
Serves as a clinical resource to nursing and other UCSF staff.
Identifies signs of acute decompensation or failure and communicates promptly with the physician.
Participates in service rounds, patient evaluations, and treatment planning as part of the multidisciplinary team.
Reviews labs, imaging, and other diagnostics to guide care plans.
Evaluates complex cases to determine appropriate level of care.
Sets discharge parameters in coordination with the broader care team.
Coordinates care across the inpatient and outpatient setting to ensure safe discharges and effective communication and care transfer between settings.
Responds to urgent weekend calls for critical symptoms or lab results.
Remains available to address time-sensitive patient issues outside normal hours.
