Nurse Practitioner (PRN) - In-Home Health Assessments

Matrix Medical Network•Wills Point, TX
•Onsite

About The Position

Matrix Medical Network is hiring a PRN Nurse Practitioner to conduct in-home Comprehensive Health Assessments (CHAs) for adults and older adults, including people who may be at risk of undiagnosed chronic conditions. Visits may last up to one hour and take place at members’ residences, including private homes and Skilled Nursing Facilities. Providers review and observe members’ health, medical history, medication adherence, social environment, and other risks. The assessments provide a broader view of members’ well-being than may be available during routine office visits and can help improve care quality and close multiple care gaps in a single visit. The role offers scheduling flexibility: visits are scheduled around provider availability, with assessments conducted Sunday through Saturday, from 8 a.m. to 7 p.m. Compensation is based on completed assessments; providers completing an average of 20–100 visits per month may earn $1,800–$11,500 per month, at a rate of $90–$115 per assessment depending on state of residence. The company provides onboarding, a preceptor, individual feedback, and access to continuing education. PRN providers may transition to full-time roles.
