The Advanced Practice Clinician will provide and coordinate primary care to SCAN members to achieve the Company’s mission to keep seniors healthy and independent. This is a field-based role covering San Bernardino, CA, including Ontario and Chino Hills. The role involves providing care to an assigned panel of members in their place of residence, typically senior living communities or skilled nursing facilities. Responsibilities include conducting physical/wellness exams and assessments, occasional urgent medical visits, telephonic triage, administering vaccines, ordering and interpreting diagnostic tests, initiating goals of care conversations, and chronic disease management. The clinician will identify patient healthcare needs, develop comprehensive plans of care, and collaborate with case management for timely access to services. Independent management of visitation schedules, engagement with Primary Care Physicians and other healthcare professionals, and partnership with specialists, hospitals, and community-based organizations are key. The role also involves monitoring and addressing clinical quality gaps, documenting visits, serving as a direct resource for members and families, participating in sales team events, initiating virtual health visits, and participating in an after-hours on-call program. Continuous professional development and contribution to team efforts for quality healthcare goals are expected. This is a patient-facing role, and employees may be eligible for differential pay.
