About The Position

Circadia Health is seeking an on-site Nurse Practitioner (NP) to transform bedside care with AI. In this role, you will be the first to see patients, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions, ultimately reducing preventable readmissions. You will blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments. This is a full-time, embedded NP role requiring Monday-Friday, on-site presence at our partner facility in Pasadena, TX.

Requirements

• Certified and licensed Nurse Practitioner in TX; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver's license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required.

Nice To Haves

• Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.

Responsibilities

• Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Complete documentation/charts within the same day.
• Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Surface field insights to improve workflows and tech integration in a clinical setting.
• Support NP onboarding, share best practices, and partner with clinical ops/product teams.
