About The Position

The University of Rochester is seeking an experienced Nurse Practitioner (NP) to join a busy Family Medicine practice in Batavia, NY. This full-time position is part of the UR Primary Care – Batavia network and operates within the Patient Centered Medical Home model. The practice is a fast-paced, semi-rural, community-based setting serving a diverse patient population, including Spanish-speaking patients (language line services available). The providers care for patients across all age groups, from newborns to older adults, managing a wide range of medical issues. The role emphasizes interdisciplinary team collaboration and offers opportunities for professional growth within a leading research university committed to the value of Meliora - Ever Better.

Requirements

  • Master's degree as a Nurse Practitioner.
  • Active New York State Registered Nurse license in good standing.
  • Active New York State Nurse Practitioner licensure in good standing.
  • Current national certification as an Nurse Practitioner.
  • NPs will demonstrate clinical competence, superior verbal and written communication skills, interpersonal effectiveness and customer relation skills, computer competence and must have an active desire to learn.
  • Greater than 2 years of active practice is required.
  • NYS NP certification along with licensure.
  • Must have applied for DEA registration prior to hire/start date.

Nice To Haves

  • An NP is favored.
  • Spanish-speaking ability is a plus.

Responsibilities

  • Evaluation and management of acute and chronic illnesses.
  • Ordering, interpreting, and communicating diagnostic tests and results.
  • Performing complete physical exams and wellness visits.
  • Conducting hospital/ED follow-ups and transitional care management.
  • Collaborating with network-based physicians and the medical team regarding patient care.
  • Facilitating appropriate referrals to specialists or community-based agencies.
  • Opportunities for office-based procedures.
  • Providing patient education and counseling to maintain or restore physical/emotional health and function.
  • Offering educational support for various learners consistent with the goals of the UR teaching institution.
  • Establishing a team-supported patient panel.
  • Participating in the education of learners (students and new employees) through clinical learning experiences, knowledge sharing, and skill training.
