About The Position

MOSAIC Community Clinic (MCC) is opening in the Burnaby Edmonds neighbourhood in February 2025 and is looking to hire Nurse Practitioners with flexible offerings. The clinic operates within the Burnaby Primary Care Network (PCN), which is part of BC’s Primary Care Strategy focusing on team-based care. MCC provides primary care services including routine check-ups, immunizations, chronic disease management, women’s and children’s health services, specialist referrals, and patient education. The clinic offers culturally safe, trauma-informed healthcare to diverse priority populations, including newcomers, Indigenous peoples, new mothers and their babies, individuals with complex mental health conditions, and those experiencing homelessness or precarious housing. The clinic is a bright, brand-new facility with 8 exam rooms and 2 consult rooms, offering a collaborative environment with family physicians, social workers, lactation consultants, registered nurses, and administrative support.

Requirements

• Masters Degree - Nurse Practitioner
• ALCS
• BLCS
• CPR
• Driver's License

Responsibilities

• Provide high-quality primary care to diverse patient populations.
• Collaborate with a multidisciplinary team to ensure wrap around care.
• Engage in trauma and violence informed longitudinal primary care services.
• Adhere to the principles of a Patient Medical Home, ensuring patient-centered care.
• Provide outreach services as needed.
• Coordinate care with specialized health services, hospitals, and other community services.
