About The Position

Ohio State University Physicians (OSUP) is seeking a Nurse Practitioner to serve as a primary clinical lead within our Skilled Nursing and Sub-Acute Rehabilitation unit. This role is designed for a clinician who excels in a senior living setting, managing complex medical transitions for residents moving from acute hospital care to rehabilitation. The Nurse Practitioner will provide outpatient care to geriatric patients in a clinic located in Dublin, Ohio.

Requirements

Active Ohio APRN license and national certification (AANP or ANCC).
Minimum of 2 years of experience in hospitalist medicine, geriatrics, or sub-acute Rehab preferred.
Proficiency with PointClickCare or similar EMR.
Strong autonomous decision-making skills.
Cohesive collaborator and communicator.
Ability to thrive in a high(er)-volume, fast-paced environment.

Responsibilities

Provide outpatient care to geriatric patients in a clinic located in Dublin, Ohio.
Collaborate on patient care with physician, therapy, nursing, and social work.
Order and interpret diagnostic tests, manage medication reconciliation, and perform minor procedures (i.e. stitch removal) as needed.
Perform comprehensive evaluations (H&P, progress notes, (re)certifications).
Partner with the Medical Director to ensure all documentation meets CMS Five-Star Quality Rating requirements and supports regulatory readiness.
Document all patient encounters accurately and timely in the electronic medical record (EMR).
Act as the primary clinical liaison for families, communicating effectively with patients and families to align medical interventions with goals of care.
