About The Position

Ohio State University Physicians (OSUP) is seeking a Nurse Practitioner to serve as a primary clinical lead within our Skilled Nursing and Sub-Acute Rehabilitation unit. This role is designed for a clinician who excels in a senior living setting, managing complex medical transitions for residents moving from acute hospital care to rehabilitation.

Requirements

• Active Ohio APRN license and national certification (AANP or ANCC).
• Minimum of 2 years of experience in hospitalist medicine, geriatrics, or sub-acute Rehab preferred.
• Proficiency with PointClickCare or similar EMR.
• Strong autonomous decision-making skills.
• Cohesive collaborator and communicator.
• Ability to thrive in a high(er)-volume, fast-paced environment.

Responsibilities

• Provide outpatient care to geriatric patients in a clinic located in Dublin, Ohio.
• Collaborate on patient care with physician, therapy, nursing, and social work.
• Order and interpret diagnostic tests, manage medication reconciliation, and perform minor procedures (i.e. stitch removal) as needed.
• Perform comprehensive evaluations (H&P, progress notes, (re)certifications).
• Partner with the Medical Director to ensure all documentation meets CMS Five-Star Quality Rating requirements and supports regulatory readiness.
• Document all patient encounters accurately and timely in the electronic medical record (EMR).
• Act as the primary clinical liaison for families, communicating effectively with patients and families to align medical interventions with goals of care.
