About The Position

Relive Health provides personalized, proactive care focused on understanding the client as a whole. Through comprehensive testing, clinical evaluation, and individualized treatment planning, we help clients improve how they feel, function, and live. Our services include hormone optimization for women and men, functional health, medical weight management, peptide therapy, advanced diagnostic testing, IV therapy, regenerative medicine, aesthetics, and longevity-focused care. We are seeking a clinically confident Nurse Practitioner or Physician Assistant to join our Pennsylvania team. This position is best suited for a provider who already has experience in one or more of the following areas: HRT/TRT, women’s hormone health, medical weight management, peptide therapy, metabolic health, or functional/integrative medicine. Relive provides established protocols, clinical resources, education, pharmacy support, and collaborative medical guidance. However, we are specifically looking for a provider who brings an existing clinical foundation and is comfortable evaluating patients, interpreting laboratory testing, developing treatment plans, prescribing within their scope of practice, and monitoring patient response.

Requirements

Graduate of an accredited Nurse Practitioner or Physician Assistant program
Active, unrestricted Pennsylvania license and current national certification
Current prescribing authority and required collaborative or supervisory agreements
Ability to obtain and maintain DEA registration if required
Strong understanding of patient selection, contraindications, informed consent, monitoring, and follow-up
Strong clinical judgment, communication, teamwork, and technology skills
Comfortable using EHR, laboratory, pharmacy, and prescribing platforms

Nice To Haves

3+ years of relevant clinical experience preferred
Prior hands-on experience with HRT/TRT, women’s hormone therapy, medical weight management, peptide therapy, metabolic health, or functional/integrative medicine strongly preferred
Experience interpreting hormone and metabolic laboratory testing and monitoring treatment plans preferred
Aesthetic injectable experience is a plus

Responsibilities

Conduct comprehensive patient evaluations, physical examinations, lab reviews, and follow-up visits
Review symptoms, medical history, medications, lifestyle, goals, risks, and diagnostic findings
Interpret comprehensive bloodwork and other diagnostic testing
Develop individualized, evidence-informed treatment plans
Evaluate, prescribe, and manage hormone optimization for eligible women and men
Evaluate women experiencing perimenopause, menopause, and other hormone-related concerns
Prescribe and manage medical weight-loss therapies, including GLP-1 medications when clinically appropriate
Evaluate and manage peptide therapies offered within Relive’s clinical and compliance framework
Monitor symptoms, laboratory results, progress, treatment response, and side effects
Adjust treatment plans based on ongoing clinical evaluation
Educate patients regarding treatment options, risks, benefits, expectations, and follow-up
Recognize findings requiring additional evaluation, referral, or escalation
Collaborate with physicians, clinical staff, and pharmacy partners
Complete accurate and timely documentation
Maintain HIPAA, safety, infection-control, and compliance standards
Participate in continuing education and Relive training
