Requirements

  • Completion of Physician Assistant or Nurse Practitioner accredited program.
  • Active, unrestricted license to practice in Louisiana.
  • At least 2 years of experience providing clinical services to pediatric (including newborns) and adult patients with both chronic medical and behavioral health conditions.
  • Interest in serving underserved, vulnerable populations.
  • Demonstrated proficiency, prior experience, and/or willingness to train in clinical nursing skills such as blood drawing (venipuncture & phlebotomy), patient assessments, and care plan reinforcement for common adult and pediatric chronic conditions.
  • Proven skills, knowledge base, and judgment necessary for independent clinical decision-making and problem-solving.
  • Available to work a full-time, five-day work week.
  • Participate in a rotating call schedule that includes occasional evening and weekend days each month.
  • Maintains current CPR certification (BLS).
  • Access to reliable transportation and willingness to comply with all the terms of the Nest Motor Vehicle Safety Policy.
  • This position will involve educating and encouraging vaccination against COVID-19.

Responsibilities

  • Manage a panel of families as the primary care provider to improve whole-family health through longitudinal primary and preventive care, care management, and care coordination in the home setting and telehealth and virtual care modalities.
  • Practice with the support of a robust interdisciplinary team, including MDs, LCSWs, RNs, psychiatrists, dieticians, and medical assistants.
  • Utilize Nest Health’s electronic health record and care management tools to collect data, document member interactions, organize information, track tasks, and communicate with team, members, and community resources.
  • Attend daily team huddles and weekly case conferences to advise on physical health needs and establish health goals as part of member’s NAP (Nest Action Plan).
  • Propose treatments for chronic and infectious diseases, prescribe and reconcile appropriate medications, and educate patients about their conditions and medications.
  • Order, administer, obtain, and record diagnostic tests.
  • Provide information and assistance through advocacy, navigation support, and education to Nest families on the availability and eligibility of health and social care services.
  • Participate in outreach efforts to Nest families, including explaining program details to potential families.
