About The Position
Altais is seeking a Nurse Practitioner to join their fast-growing, dynamic team. This role involves performing comprehensive health assessments, including obtaining history, physical examinations, reviewing medications, and assessing psychosocial needs. The Nurse Practitioner will be responsible for fulfilling documentation requirements for STAR and HCC measures, obtaining and documenting all pertinent patient information, including medications and end-of-life preferences, and instructing patients and families to call prior to accessing medical care (except in emergencies). Maintaining patient safety, confidentiality, and utilizing universal precautions are essential. The role also includes prescribing/ordering procedures, treatments, and medications within the scope of practice, and developing a high level of trust and intimacy with patients and families.
Requirements
- CA Licensed Nurse Practitioner
- 1+ Years of Experience with Primary Care
Responsibilities
- Perform comprehensive health assessment, including obtaining history and performing physical examination.
- Complete review of medicines and related instructions.
- Review home safety and psychosocial needs.
- Fulfill documentation needs for STAR and HCC measures.
- Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions.
- Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies.
- Maintain patient safety and confidentiality.
- Universal precautions utilized in all patient care.
- Prescribe/order procedures, treatments and medications within scope of practice.
- Develop high level of trust and intimacy with patients and families.
