Requirements

Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
Active, unencumbered NP license and Board Certification (ANCC or AANP).
Valid driver’s license and vehicle for local field-based travel.
Applicants must be legally authorized to work in the United States without the need for employer sponsorship now or in the future.
Successfully complete a pre-employment drug screening.
Successful completion of a background check, as permitted by law.
Satisfactory results of monthly FACIS (Fraud and Abuse Control Information System) checks.

Responsibilities

Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient’s home.
Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.
Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.
